What data we require

Haly Dental Care will be the ‘controller’ of your personal data. Haly Dental Care’s company registration number is 33077180H and it’s registered address is 35A Mary Street, Cork City. Unless otherwise agreed with you, we will collect basic personal data (name, phone number, address, PPS number, date of birth and email address) as well as sensitive personal data ( Medical history, dental records, x-rays, medical correspondence etc)

Why we need it

We need to know both basic and sensitive personal data in order to provide you with the dental services you have engaged us to provide and to assert our right to be recompense for these services, in line with the agreement or contract we have with you, our patient. If you do not provide this information, we will not be in a position to treat you at our practice. We will only collect data that is relevant to the provision of your dental treatment, and to effectively communicate with you about this treatment, and nothing more

What we do with it

All the personal data we hold about you will be processed by our staff in Ireland. There are also some third parties that may have limited access to your data for processing purposes. They are as follows:

Your data may also be provided to third parties where there is a legal obligation to do so. Where data is backed up remotely, this data is kept on services located within the European Union. We take all reasonable steps to ensure your data is processed discretely and kept securely.

How Long We Keep It

In line with the Dental Council of Ireland’s Code of Practice in relation to Professional Behaviour and Ethical Conduct, we will keep adult patient records (both basic and sensitive data) for eight years after the date of their last treatment. In the case of children and young adults, records will be kept until the patients 25th birthday; or their 26th birthday if the young person was 17 whenthey finished treatment. If a patient dies before their 18th birthday, records will be kept for eight years. After this time the records will be safely destroyed if it is no longer required for the lawful purpose (s) for which it was obtained.

What Else We May Use It For

Separate to communication relating to your treatment at our practice, we would like to send you newsletters and/or information about new services from time to time, by post, email and SMS. This consent will be obtained from you separately.

Practice Communication Consent

At Haly Dental Care, we take your privacy seriously. From time to time we would like to contact you with details of dental services that we provide or introduce, such as additional dentists/treatments or introductory treatment offers. These communications may be via post, email or SMS. We will never pass your details on to other parties for unsolicited marketing purposes. You also have the right to withdraw your consent and to opt out of this communication at any time. If you consent, your data will be used for the purposes outlined below only:
